Walk to wellbeing form

If you’d like to take part in the Thursday walk to wellbeing sessions, please complete the Par-Q form below. Anyone who wants to take part in the session, including anyone you bring with you must complete the form.

Once the form has been completed it will be sent automatically to the student achievement team, and you will receive a confirmation email. You will not be able to participate without completing the form. 

  • PAR-Q form
    PAR-Q is designed to help you help yourself. Many health benefits are associated with regular exercise, and the completion of PAR-Q is a sensible first step to take if you are planning to increase the amount of physical activity in your life. For most people, physical activity should not pose any problems or hazard. PAR-Q has been designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them. If you are considering increasing the amount of physical activity you do on a regular basis, then please complete the below form.
